Thanks a lot for responding my post in this short period of time.
I am glad to learn that reboot the Apple TV solves the issue in your environment. To my system, that doesn't work.
However, it seems like a mystery.
But good luck, I bought the new ATV 4K (only) in advance of the possible ATV 4 HDs end of lifecycle one day in the future. Right now the older one still works (perfect).
If I should have a lot of time during the next weeks, I will test the new ATV 4K with another TVHeadend-Backend. I guess that the more power of this new device overwhelms my current backend. But, as you surely know, setup a new backend is another nightmare as well.

Today I want go get back to this (to me) unsolved problem.
After having installed another setups of TVHeadend-backends, there was no solving to the problems.
So, I decided to struggle and figure out all options and settings in MrMc-app. Following this, I guess that I have found the reason and solution for that:
With these settings, the skip forward/backward works perfectly.
Maybe, this could help someone of you.

In "Settings/Video/Acceleration (German Beschleunigung) I deactivated "Hardware acceleration for Video-Toolbox" and "Hardware acceleration - AV Foundation".
"Disable limited to full video rescaling" is turned on.
With these settings, the skip forward/backward works perfectly.![]()

Just thought I'd chime in since I've come across this same issue as well recently. Happens on both a regular ATV4 and 4K. I did the suggested restart from the Settings menu and now fast forward & rewind work correctly via side clicks. Previously the OSD menu would come up and the video would pause when trying to side click fast forward. I'll try to keep an eye on how long it takes until next occurrence and see if I can get a log debug file.
